Our Treatment Goal

Although it is unfortunate that patients develop renal failure and require dialysis therapy, our goal is to “extend the duration of healthy living as much as possible.”

To more effectively remove metabolic waste products that accumulate in the body, it is necessary to increase the dialysis dose (dialysis time and blood flow rate). However,standard hemodialysis already lasts approximately four hours, and further extension can impose a significant burden on patients.

At our clinic, we extend dialysis time by an average of approximately 20 minutes and increase the blood flow rate from 200 mL/min to 280 mL/min (approximately 1.4 times).

This approach theoretically improves the efficiency of waste removal by about 1.5 times.
Through this strategy, metabolic waste products and uremic toxins can be more effectively eliminated, which is expected to help maintain and improve overall physical condition (energy levels and well-being).

We aim to provide “time-intensified dialysis,” supporting patients in maintaining a condition closer to good health over a longer period.

Key Features of Our Clinic

1. Since the era of Shio Clinic (1977–2011), we have consistently emphasized comprehensive systemic management.
This is reflected in patient survival rates, which are favorable compared to the national average (high-quality dialysis).

2. We actively provide online hemodiafiltration (HDF), which is associated with less intradialytic hypotension and superior removal of amyloid-related substances.
Currently, approximately 80% of our patients receive this therapy, and it is also effective for patients with severe pruritus (itching).

3. We primarily use acetate-free dialysis fluid (Carbostar), which is associated with reduced post-dialysis fatigue.

4. To prevent complications such as dialysis-related amyloidosis, we employ advanced systems to achieve high-level purification of dialysis fluid.
Our water quality meets the standards recommended by the Japanese Society for Dialysis Therapy.

5. In cases of clinical deterioration or when hospitalization is required, we collaborate closely with regional general hospitals.(Open system: our physicians provide co-management together with hospital-based physicians.)

6. Although located in a central urban area, we provide sufficient parking space for patients who commute by car.
(We do not offer transportation services. Walking to the clinic is considered an important form of rehabilitation to maintain lower limb muscle strength. Loss of muscle strength may lead to frailty and negatively affect prognosis.)

7. We implement exercise therapy during dialysis sessions to promote physical activity.

8. For patients who have difficulty visiting the clinic regularly for dialysis, we offer peritoneal dialysis (PD), which can be performed at home and is also suitable for palliative care and end-of-life care.

Examinations for Dialysis Patients

1. Bone density testing for the diagnosis of bone disorders (once every 6 months)

2. Ultrasonography using advanced equipment for early detection of complications
(once per year)

3. Vascular access (shunt) angiography to maintain and protect the access (as needed)

4. Assessment of shunt blood flow using high-performance ultrasound Pulse wave analysis to evaluate arterial stiffness (once every 6 months)
Additional lower limb vascular ultrasound may be performed if abnormalities are detected

5. Foot care for early detection of foot lesions

Body composition analysis (using InBody S20) for accurate dry weight assessment Enables measurement of not only body fluid but also muscle mass, and evaluation of exercise effects In addition, portable ultrasound is used during dialysis to assess inferior vena cava diameter and monitor fluid status.

We place a strong emphasis on staff training.

We hold a monthly study session where staff learn about the medications used in dialysis treatment and work to improve their dialysis techniques.

We also make various efforts to help maintain and improve staff motivation. For example, we provide strong financial support for staff participation in academic conferences and professional meetings.

Although we practice team-based medical care as a unified organization, we also divide staff into several smaller teams. This allows staff members to quickly consult within their own team when they are unsure how to assess or respond to a patient’s condition.

Dialysis patients often face a wide range of physical and emotional challenges. Caring for these patients can place significant psychological stress on nurses and other medical staff. To address this, we regularly provide mental health support for our staff through an external professional service.

In addition, while the nationally recommended number of annual holidays is approximately 120 days, we encourage staff to take around 140 days of paid leave per year. This helps reduce work-related stress and supports a healthier working environment.
